Weight loss and willpower
I'm desperate to lose weight but I just can't seem to diet. I've no willpower at all and can't resist temptation. I've dieted many times before and know I can do it (although the weight always seems to come back) but this time I just can't find the motivation. I'm so fed up.

I'm convinced the key to successful weight loss lies totally in our brains, not our stomachs. Losing weight should be simple - you need to eat fewer calories/fat and burn more up through increasing your activity levels.
It doesn't really matter what those calories/fat are, although you should aim for healthy eating that you can maintain as a long-term part of your lifestyle, rather than a quirky diet that you can't keep up.
Hard to get your brain into gear
What's hard is getting your brain to do this - as you say, the motivation or willpower factor. I've often thought there's huge scope for personal weight loss trainers, like gym trainers, who help you get motivated and tone your brain up for dieting.
There are one or two good general books on the psychology of weight loss, which suggest all sorts of tips and 'brain plans'. Check your local library or bookshop.
In addition, you should consider the following:
Joining a slimming group - many people find that group work, with regular weigh-ins and sharing of experiences, boosts their motivation
Formulate your own 'brain plan' and write it all down - map out the next few months by marking out target weights, days off from your diet and rewards for progress
Learn your own weaknesses, failings and food triggers, and look for ways to beat them - try to avoid getting too tired or going too long without food
Tell people you're dieting - you might bore them, but it helps you to commit to the idea
Weigh yourself once a week - more often and you'll see no improvement and get disillusioned
Don't be overambitious - a loss of 2lb or maximum 1kg a week is the most you should expect
Anticipate the hurdles and falls - if you know you're going to go away for a weekend, for example, prepare tactics for eating out, anticipate a slight gain in weight and mentally prepare to get back on track as soon as you return
All dieters have bad days and times - mentally plan small treats or tactics to cope for when life is difficult

For decades, dieters and doctors alike believed willpower was the key to successful weight loss.
"Many people go through life believing that they can't stick to a diet because they have no willpower. They believe that some innate force is keeping them from resisting food temptations," says Warren Huberman, PhD, a psychologist who counsels patients in conjunction with the New York University Program for Surgical Weight Loss.
The truth, experts now say, is that the ability to stick to a weight loss diet has little to do with will -- and everything to do with changing the way we think about food.
"People like to think of willpower as some mystical, magical power over which they have no control … but in reality there is no such thing," says Gerard Musante, PhD, founder and director of Structure House, an inpatient weight loss program in Durham, N.C.
Believing that willpower is at work only serves to make you feel less in control of your eating habits, experts say.
"Once you buy into the idea that self-control is something that is out of your control, and the domain of some indigenous character trait that you either have or you don't, then on some level you accept that staying on a diet is also not within your control, " says Huberman.
Understanding Temptation
When formal studies of weight loss got under way in earnest in the 1950s, willpower was the basis for most diets. Patients who were unsuccessful at losing weight were told by their doctors to simply eat less -- and those who couldn't were labeled as having no "willpower."
But by 1967, research conducted at the University of Michigan began to change everything. It was here doctors discovered that the ability to lose weight wasn't rooted in willpower at all -- but instead, in simple behavioral changes.
When patients were taught how to substitute a fulfilling activity for filling up with food, their appetites, and eventually, their weight, became easier to control.
Those findings went on to form a major focus of weight loss today -- behavior modification. Experts now believe that sticking with a diet isn't about willpower, but about the ability to understand and ultimately change behaviors linked to how and why we choose the foods we eat.
"If you find yourself resisting a food you really want to eat, that's not willpower, that's a choice – and within each of us lies the power to make these choices about everything in our life, including the food we put on our plate," says Huberman.
While it might sometimes seem as if you're being overtaken by an overwhelming desire to eat a certain food, what you're really experiencing, says Huberman, is an unconscious behavioral reaction -- or sometimes, just a bad habit.
"In either case, it's important to realize it's not some mystical force of the universe that's controlling your desire to eat something, it's your own behavior -- which also means it's something that can be changed," says Musante.
The Biology of Willpower
If unconscious behavior is one dark angel sitting on your shoulder and whispering "eat that now!" many experts believe that innate biological drive is another. Our need to eat is far stronger than any sense of "willpower" that could exist.
"Eating is a basic human need. We all need a certain amount of food to survive, and our brain and our body is hardwired to overcome any obstacle that stands in the way of that survival," says psychologist Abby Aronowitz, PhD, author of Your Final Diet.
Indeed, Aronowitz says, possibly the strongest biological drive we have is to satisfy our hunger. And the greater the hunger, the stronger the drive.
"When incentive is high, we can put off our needs temporarily and sustain for a while on our reserves," she says. But sooner or later, hunger kicks in. And when it does, what we perceive as our willpower goes out the window.
"At some point in time, any diet which is too restrictive is going to allow that natural biological drive to eat to take over, meaning the floodgates are open and defenses are down," she says.
So while grabbing that huge donut off the coffee cart may make us feel weak-willed, what we really are may be hungry, says Aronowitz. Resisting that temptation, she says, may be as simple as getting adequate nutrition, and finding an eating plan that doesn't leave you starving.
Additionally, evidence continues to mount that certain hormones may also be influencing what may seem like willpower. Aronowitz points to evidence indicating that when estrogen levels rise -- as they do just before ovulation in women -- it can increase the desire for certain foods, including chocolate.
Likewise, a drop in levels of serotonin -- a brain chemical linked to depression -- could signal an increased desire for carbohydrates.
As we learn more about the role of hormones linked directly to appetite, like ghrelin, leptin and cortisol, we'll also likely discover more about how imbalances can increase our cravings and make resisting some foods especially hard, Aronowitz says.
"What can seem like willpower may, in fact, be biology at work," she says.
Finding Your Inner Strength
While doctors don't always agree on just what is behind a person's ability to resist tempting foods, one belief that seems universal is that we all have such an ability.
For many, the key lies in understanding their tolerance level for "food frustration," and the ability to plan ahead for how to handle it.
"If you know, for example, that you'll be at a holiday party with lots of tempting treats, going into the event relying on nothing but your willpower almost always guarantees disaster," says Huberman.
But going in with a plan for how you'll handle the temptation will give you a sense of control -- that feeling of "willpower" to resist what you really don't want to eat.

For example, highly restrained dieters are very precise about how much they eat. You may be surprised to learn that a very high level of dietary restraint is associated with obesity, not with successful weight management, and also is linked to lack of success in weight-loss attempts. How can high levels of dietary restraint be connected with excess weight? While all of the specifics are an intense area of scientific investigation, the simple answer is that high restraint levels cannot be sustained for extended periods of time. Invariably, the restraint is abandoned and overeating occurs. The flipside of dietary restraint is dietary disinhibition, which means lack of inhibition when it comes to eating. High levels of dietary disinhibition also are associated with obesity and lack of weight-loss success.
Restrain, But Be FlexibleIf dietary restraint is linked with overweight and unsuccessful weight loss, and dietary disinhibition likewise is linked with overweight and lack of success, what is the best way to be? The answer is to find the middle ground, with a strategy called flexible restraint. Flexible restraint means putting a moderate level of control on your eating. Your eating plan needs enough structure to give you the security and comfort that you are in control of your eating. On the other hand, the plan needs enough flexibility so that you won't feel trapped, deprived or restricted by overly strict rules. The key is to find the balance between control, or restraint, and flexibility in an eating plan that is not so rigid that it is impossible to stick with.
Willpower versus "want power" Managing your weight means finding the balance between willpower and "want power." If willpower is defined as the mental strength to set limits for yourself, "want power" is your desires and motivation, the forces that drive you to behave in a certain way. "Want power" is not just about food; it is about your entire weight management strategy. Before you select a weight management program, have a talk with yourself about what you want (want power) and don't want (will power). Select a program that will help you be successful by satisfying your wants (within reason, of course) without requiring superhuman amounts of willpower.
Motivations ChangeHere is a really surprising research finding – how motivated a person is at the beginning of weight loss has nothing to do with whether weight loss will be successful. We all know people who are very gung-ho when they decide to lose weight. And we are shocked when they do not achieve lasting results. What happened? The explanation is that there is a difference between short-term and long-term motivation. Short-term motivation describes what is motivating you right now. Long-term motivation is the bigger picture. A person can be motivated long-term by the prospect of better health or by being able to deal with food in a healthier way.
Strategies Need PlansAchieving sustainable weight loss requires a map, or specific plan. While adopting a long-term mindset is very important, it also is important to break down the process into a specific plan by writing guidelines for yourself. Some people use a datebook, others prefer electronic organizers, and many go low-tech with a piece of paper or index card. You can include everything you're doing for the day: regularly scheduled appointments, appointments with doctors and dentists, and also activities related to weight management, such as exercise sessions and weekly meetings. Creating a schedule, and following it, helps you rely less on willpower to get through the day.

Getting over the hurdle of believing only in willpower and into the process of establishing a comprehensive method is critical to achieving lasting weight loss.
Holding BackLet's define one aspect of willpower as the ability to hold yourself back from eating or overeating, particularly when you're around food. Weight-loss scientists call this "dietary restraint." Dietary restraint describes how tightly a person regulates her or his food intake. For example, highly restrained dieters are very precise about how much they eat. You may be surprised to learn that a very high level of dietary restraint is associated with obesity, not with successful weight management, and also is linked to lack of success in weight-loss attempts. How can high levels of dietary restraint be connected with excess weight? While all of the specifics are an intense area of scientific investigation, the simple answer is that high restraint levels cannot be sustained for extended periods of time. Invariably, the restraint is abandoned and overeating occurs. The flipside of dietary restraint is dietary disinhibition, which means lack of inhibition when it comes to eating. High levels of dietary disinhibition also are associated with obesity and lack of weight-loss success.
Restrain, But Be FlexibleIf dietary restraint is linked with overweight and unsuccessful weight loss, and dietary disinhibition likewise is linked with overweight and lack of success, what is the best way to be? The answer is to find the middle ground, with a strategy called flexible restraint. Flexible restraint means putting a moderate level of control on your eating. Your eating plan needs enough structure to give you the security and comfort that you are in control of your eating. On the other hand, the plan needs enough flexibility so that you won't feel trapped, deprived or restricted by overly strict rules. The key is to find the balance between control, or restraint, and flexibility in an eating plan that is not so rigid that it is impossible to stick with.
Willpower versus "want power" Managing your weight means finding the balance between willpower and "want power." If willpower is defined as the mental strength to set limits for yourself, "want power" is your desires and motivation, the forces that drive you to behave in a certain way. "Want power" is not just about food; it is about your entire weight management strategy. Before you select a weight management program, have a talk with yourself about what you want (want power) and don't want (will power). Select a program that will help you be successful by satisfying your wants (within reason, of course) without requiring superhuman amounts of willpower.
Motivations ChangeHere is a really surprising research finding – how motivated a person is at the beginning of weight loss has nothing to do with whether weight loss will be successful. We all know people who are very gung-ho when they decide to lose weight. And we are shocked when they do not achieve lasting results. What happened? The explanation is that there is a difference between short-term and long-term motivation. Short-term motivation describes what is motivating you right now. Long-term motivation is the bigger picture. A person can be motivated long-term by the prospect of better health or by being able to deal with food in a healthier way.
Strategies Need PlansAchieving sustainable weight loss requires a map, or specific plan. While adopting a long-term mindset is very important, it also is important to break down the process into a specific plan by writing guidelines for yourself. Some people use a datebook, others prefer electronic organizers, and many go low-tech with a piece of paper or index card. You can include everything you're doing for the day: regularly scheduled appointments, appointments with doctors and dentists, and also activities related to weight management, such as exercise sessions and weekly meetings. Creating a schedule, and following it, helps you rely less on willpower to get through the day.

Rural Workforce Agency Victoria chief executive Claire Austin said that with an average age of almost 50, many of the 1220 rural GPs were looking for a better work-life balance in the twilight of their careers.
And with 10 per cent of them aged over 65, retirement was on the minds of others.
Ms Austin said there were already 130 vacancies for doctors in country Victoria and further dramatic change in numbers and work hours was on the horizon.
She said that simplifying the recruitment process for overseas-trained doctors -- including those from the UK, New Zealand and Asia -- would help fill the gaps.
Partnerships between rural doctors and other health professionals -- such as nurses, dentists, and mental health experts -- were also needed to relieve demands on GPs.
Ms Austin said such teams should be built around the needs of rural communities.
These and other rural workforce issues will be explored at the Sustainable Solutions in Rural Health conference, which opens today.
But the Lawlesses believe the answer lies in returning to the old days when being a rural GP was a lifestyle, part-time work was unheard of and doctors were respected leaders of the community and health industry.
"There is no such thing as a part-time doctor," Jim said.
"When you're called, you go. You don't argue the point about what time of the day it is, or what you're paid or whether you should be called."
Meg said the influx of female GPs had changed the profession, with doctors demanding family and work balance.
